TERMS & CONDITIONS

Welcome to Paragond Health, operated by Paragond LLC (“Paragond,” “we,” “our,” or “us”).

These Terms & Conditions govern your use of our website, www.paragond.health, and certain administrative aspects of our healthcare services, including scheduling, billing, payments, memberships, and communications.

By using our website, you agree to these Terms & Conditions. Patients may also be required to review and sign separate agreements, consents, and financial policies before receiving healthcare services.

MEDICAL SERVICES

Paragond provides personalized healthcare services, including hormone replacement therapy, metabolic health, weight management, thyroid care, sexual wellness, preventive care, and related services.

All medical services are provided according to the clinical judgment of licensed healthcare professionals and applicable federal and state laws. Medical evaluations, laboratory testing, prescriptions, and treatment recommendations are determined individually based on medical history, clinical findings, and professional judgment. No specific treatment, prescription, medication, or clinical outcome is guaranteed.

WEBSITE INFORMATION AND MEDICAL DISCLAIMER

Information provided on our website is intended for general educational and informational purposes and does not constitute individualized medical advice. Website content should not replace an evaluation or treatment recommendation from a qualified healthcare professional. Use of our website, including submitting an inquiry or contacting our practice, does not independently establish a provider-patient relationship.

If you are experiencing a medical emergency, call 911 or seek immediate emergency medical attention.

TELEHEALTH SERVICES

Paragond may provide healthcare services through telehealth when clinically appropriate and permitted by applicable laws and regulations. Telehealth services may include virtual consultations, follow-up appointments, medical evaluations, treatment planning, prescription management, and laboratory review. Patients must provide accurate information regarding their physical location at the time of each telehealth appointment. Services may be limited based on provider licensure, patient location, prescribing requirements, or other legal and clinical considerations. Certain conditions may require in-person evaluation, additional testing, referral, or emergency medical care. Participation in telehealth services may require separate informed consent.

APPOINTMENT SCHEDULING AND CANCELLATIONS

Patients are responsible for scheduling and attending appointments as recommended by their healthcare provider. Appointments may be subject to scheduling requirements, advance payment, cancellation policies, and applicable no-show fees. Any cancellation, rescheduling, or no-show charges will be disclosed through the applicable scheduling or financial agreement.

Paragond reserves the right to reschedule appointments when necessary due to provider availability, clinical considerations, technical difficulties, or circumstances outside our reasonable control.

PAYMENTS AND BILLING

Paragond operates primarily as a cash-pay healthcare practice. Patients are responsible for applicable charges associated with appointments, laboratory testing, medications, memberships, supplies, and other services or products they authorize. Fees may vary based on the services provided, treatment requirements, and selected payment or membership arrangements. Payment may be required before or at the time of service, according to the applicable financial agreement.

Paragond uses Stripe to process payments, manage authorized stored payment methods, and administer recurring billing arrangements. Payment processing may be subject to additional terms and privacy practices established by Stripe.

STORED PAYMENT METHODS

Patients may choose to authorize Paragond to retain a payment method on file for future transactions. When separately authorized, a stored payment method may be used for applicable appointments, laboratory testing, medications, membership charges, outstanding balances, and other agreed-upon purchases or fees. Authorization to retain or charge a payment method is governed by the patient's applicable payment authorization and financial agreements. Patients may contact Paragond to update their payment information or request changes to their stored payment arrangements.

Removing a payment method does not eliminate an outstanding financial obligation.

MEMBERSHIPS AND RECURRING PAYMENTS

Paragond may offer optional healthcare membership programs that provide access to designated services and benefits for a recurring fee. Membership pricing, included services, laboratory benefits, appointment allowances, medication discounts, billing frequency, renewal provisions, and cancellation requirements are governed by the membership agreement applicable to the patient's selected plan. By enrolling in a membership and providing the required payment authorization, patients agree to the recurring charges and billing terms disclosed in their membership agreement.

Membership services and benefits may be subject to clinical appropriateness, scheduling availability, and applicable laws. Membership enrollment does not guarantee any specific medication, prescription, treatment, or medical outcome. Changes to membership pricing or benefits will be communicated as required by the applicable agreement and law.

MEDICATIONS, PRESCRIPTIONS, AND LABORATORY SERVICES

Medications and laboratory testing are ordered when clinically indicated and permitted by applicable laws. Prescription medications are subject to provider evaluation, medical necessity, applicable prescribing regulations, and pharmacy availability.

Controlled substances, including testosterone, may be subject to additional requirements, monitoring, documentation, follow-up appointments, and prescribing restrictions. Prescription refills are not guaranteed and may require an appointment, laboratory testing, or additional clinical evaluation.

Medications may be dispensed or shipped by independent pharmacies. Pharmacy processing, availability, and shipping times may vary.

Laboratory testing may be performed through independent laboratory providers and may involve separate collection arrangements or service requirements.

REFUNDS AND CREDITS

Refund eligibility depends on the type of service or product purchased and the terms disclosed at the time of purchase.

Refunds or credits for unused services, memberships, medications, laboratory orders, or canceled appointments are subject to applicable agreements and laws.

Any applicable refund limitations will be disclosed in the relevant financial or purchase agreement.

PATIENT RESPONSIBILITIES

Patients agree to provide accurate and complete information regarding medical history, medications, symptoms, allergies, contact information, and other information relevant to their care.

Patients are responsible for following agreed-upon treatment plans, completing recommended monitoring, communicating relevant changes in their health, and attending required follow-up appointments.

Patients are responsible for maintaining current contact and billing information and reviewing communications regarding appointments, treatment, and financial obligations.

Paragond reserves the right to decline or discontinue nonemergency services when clinically appropriate or otherwise permitted by law, subject to applicable professional obligations and continuity-of-care requirements.

PRIVACY AND MEDICAL RECORDS

Paragond maintains patient medical records using ModMed, its electronic medical record system.

Personal information and medical records are handled in accordance with applicable privacy laws and our published privacy practices.

Certain services may also involve independent laboratories, pharmacies, and other healthcare or technology providers.
